Admission Process

Alberta Publicly Funded Long Term Care

At AgeCare Sagewood, we welcome the opportunity to offer personal visits as part of your decision-making process, however, all publicly funded continuing care service admissions, including Long Term Care and Supportive Living, are made by referrals from the Alberta Health Services.

To begin the admission process, you will need to request an assessment through your local Home Care Program. A Home Care health-care professional will meet with you to discuss your needs and conduct an in-home assessment that may include input from your family doctor, your family, and other health-care professionals. Together, you and your home care team will create a plan of care with the right mix of services to support your abilities and your lifestyle.

To get more information on seniors living options or to request an assessment you can:

  • Contact your local Home Care Programor
  • Talk to your family doctor, or
  • Call HEALTHLink Alberta Toll-Free 1-866-408-5465
