Our Hospice program is focused on compassionate, resident-centred care. We work in collaboration with an interdisciplinary team that is focused on supporting our residents and their families through the end of life journey. Our Hospice community offers a home-like atmosphere with diverse resources available to support the resident and their family and provide the best quality of life experience.
The LPN works as a professional member of the care team, collaborating on the delivery of resident care to meet individual needs and within his/her level of competency. He/She integrates and applies the knowledge, skills, judgment, and interpersonal attributes required to provide safe and ethical professional services to residents. The LPN is responsible and accountable to the public for his/her own practice, which is regulated and monitored.
Our opportunities for the Hospice Program LPN is an Day line 7:00am-3:15pm (0.90 FTE) including weekends on a rotation schedule.
The commissioning of the Hospice Program requires the successful candidates to be available for the position start dates and will be restricted to work only at AgeCare SkyPointe if they hold another position within Continuing Care.
